ACH Inc. provides care with unique Physical and Spiritual comfort, with meaning and dignity to our Client.
Intermittent care provided in the patient's home, SNF, Group Homes, Assisted Living or Retirement Facility.
Consists of hospice care for a minimum of 8 consecutive hours. This level of care utilized when patient develop acute medical symptoms in his/her residence which require more than routine home care for symptoms control.
Utilized for the management of acute symptoms which can not be controlled in the home. In-patient care is to be delivered in Contracted Hospitals, SNF, or Sub-Acute Facilities.
Provides a brief relief (up to five consecutive days ) for the primary caregivers. Care is to be delivered in facility settings.
Certifies the attending Physician's prognosis, leads the Hospice Team in developing a plan of care and provides consultation to the Attending Physician.
Remains the patient's primary care Doctor and works as a member of the hospice team.
Evaluates the patient's condition during regular scheduled visits. Coordinates the plan of care techniques and the use of medications and pain management.
Initiates contact with patient and family to determine needs and provides spiritual support. Works as liaison with Clergy of individual's choice. Provides assistance with funeral and memorial preparations.
Provides direct personal care and emotional support to the patient and family and coordinates changes and needs to the RN Case Manager.
Assess social service needs of the patient and family. Provides counseling, assists in coping with anxiety, fear, stress, family conflicts, financial needs, depression, and concerns about death.
Provides non-medical friendly visits and respite to patient and family in the areas of emotional support and encouragement.
Provides a brief relief for the family primary caregivers.
